The University of Illinois at Chicago’s Division of Specialized Care for Children (known as DSCC) has selected Eccovia Solutions to provide integrated, comprehensive software solutions to manage their care coordination documentation for the youth and children served by DSCC.

DSCC was established in 1937 and helps to guide families through their child’s journey with medical conditions. DSCC works with schools, doctors, and other community groups to help create coordinated support systems for each child. DSCC also includes families in the development of care plans focused on the family’s goals for their child. This guided assistance helps families feel more organized and confident in the care of their child.

The American Academy of Pediatrics has recently highlighted DSCC for the care coordination team’s use of assessments and child-centered plans to help families reach goals, mitigate risks, and have their needs met. These assessments address a family’s needs in the following areas:

• Education
• Overall health
• Emotional health
• Social health
• Transitional needs
• Financial situation

The new ClientTrack Care Coordination platform will help bridge the gap between community providers and primary care providers with an application for data sharing, care-tracking, monitoring, reporting, and advanced analytics. ClientTrack uses prebuilt capacities for advanced care coordination and will be used by DSCC to assess the unique needs of each family, formulate child-centered plans to address these needs and connect families to the resources that will help them meet these needs.
